This webinar is designed to encourage a discussion about problems with incident investigations,
investigation bias, the limitations of 5-Whys, what goes wrong during interviews, and the importance of
a root cause analysis process.  Ken Reed, VP of System Improvements, and Nancy Hitchins, MPH, CSP,
owner of NPH Solutions LLC, together having over 50 years of incident investigation and root cause
analysis, will be presenting together, and look forward to a very interactive meeting.

Speaker Bio:

Nancy Potts Hitchens, MPH, CSP – Owner, NPH Solutions, LLC:

Nancy has over 35 years of professional experience in Environment, Health & Safety management systems.

Nancy’s career has allowed her to provide EHS leadership in multiple industries, including chemical, aerospace, oil and gas, mineral and coal mining, construction, fossil and renewable energy generation, and cement.

Nancy holds a Masters in Public Health, and has been a Board Certified Safety Professional for over 25 years.  Combining technical knowledge, years of experience, and solution-oriented focus has allowed Nancy to provide sustainable EHS solutions to global organizations.

She has been a contract consultant/trainer for TapRooT®  and training auditors in ISO 14001, ISO 45001, and ISO 9001 since 2015.

Ken Reed – Vice President, System Improvement, Inc.

Ken brings 21 years of US Navy nuclear operator and supervisor experience to System Improvements, having served on numerous nuclear-powered fast attack, ballistic missile, and research submarines. His experience in mechanical and electronic system operation, integration, and maintenance has made him uniquely suited for his current role as the Project Manager for the Equifactor® Equipment Troubleshooting module of the TapRooT® software, overseeing course and software updates and instructor certifications. He holds a B.S. in Electronics Management from Southern Illinois
University. He’s been working at System Improvements for 15 years.
